Booking a Matrix Assessment
1 When you book a Matrix Assessment we will take a £100 non-refundable deposit, charged to your Debit/Credit card.
2 We will send you a pack of information and enquiry forms which you, your child and teachers are asked to complete and return to us.
3 When we confirm your child's place at are particular session we will charge a further £100 to your card. This is non-refundable but we will endeavor to offer an alternative session if you need to cancel at this point. (Depending on the notice given you may have to pay an admin/cancellation charge.)
4 When we have completed the report we will charge the balance to your Debit/Credit card as it is sent to you.
5 We will not charge for the follow-up consultation with yourself and/or the school but any further services offered by Lucem Ltd. will need to be agreed and charged for as they are requested/ provided. N.B. VAT!!
